Resumen
Fundamentos: La infección por el virus de la hepatitis A y el de la hepatitis B continúan siendo un problema de salud pública mundial a pesar de los avances realizados hasta la fecha. La incidencia de estas infecciones varía de unos países a otros en función del control ejercido sobre los diferentes mecanismos de transmisión y el grado de instauración de la vacunación preventiva. España se figura como una zona de baja endemia de estas hepatitis, y son consideradas enfermedades de declaración obligatoria desde los años noventa. A pesar de que se ha conseguido disminuir la incidencia de estas hepatitis gracias a la implantación de los programas de vacunación, todavía son responsables de una morbimortalidad elevada. Sin embargo, aunque durante la última década se observa una tendencia decreciente, en el último año, la incidencia de ambas hepatitis ha aumentado. El objetivo de este estudio es comparar la incidencia de la hepatitis A y de la hepatitis B en Galicia y en cada una de sus provincias durante el periodo 2007 al 2016. Método: Estudio descriptivo de tipo observacional retrospectivo. Se estudiaron los casos nuevos de hepatitis A y de hepatitis B en Galicia y en cada una de sus provincias, que fueron notificados a la Red Nacional de Vigilancia Epidemiológica, desde el año 2007 al 2016. A Través de la aplicación EDOs Web se accedió a los datos sobre los casos nuevos anuales de Galicia y de sus provincias. Mediante el programa Excel, se compararon los datos y la incidencia acumulada se calculó como la diferencia entre los casos nuevos y la población promedio desde el año 2007 al 2016. Resultados: La incidencia de la hepatitis A y de la hepatitis B en Galicia ha sufrido variaciones a lo largo del periodo de estudio. A pesar de que la incidencia de hepatitis B es mayor que la incidencia de hepatitis A, ambas coinciden en un aumento del número de casos en el último año. A nivel provincial, la hepatitis B tiene mayor incidencia que la hepatitis A en todas las provincias excepto Lugo, donde la incidencia es mayor de hepatitis A que de hepatitis B. En cuanto a la incidencia acumulada, el mayor riesgo de desarrollar hepatitis A es en A Coruña, y el mayor riesgo de desarrollar hepatitis B es en Pontevedra. Conclusiones: Durante el último año se ha observado un incremento de la incidencia de hepatitis A y hepatitis B en Galicia y en sus provincias. Los brotes de hepatitis A pueden estar relacionados con personas que tienen relaciones sexuales con una persona infectada (sobre todo HSH), viajes a zonas de alta endemia sin inmunización previa, o contagios en contactos convivientes o familiares (incluidas guarderías). Por otro lado, el aumento de la incidencia de hepatitis B puede deberse también a personas que mantienen relaciones sexuales con una persona infectada y no utilizan medidas de protección y a la reutilización de agujas y jeringas (tatuajes, consumidores de drogas por vía intravenosa...).
Abstract
Basics: The infection through Hepatitis A and Hepatitis B virus continue to be a global health problem despite all the progress made so far. The incidence of these infections is different from one country to another depending on the control made over the level of setting up of the preventive vaccination. Spain is considered as an area of low endemicity of these Hepatitis, and they are considered notifiable diseases since the nineties. Although the incidence of this Hepatitis has been reduced thanks to the implementation of vaccination programs, they are still responsible for high morbidity and mortality. However, although a decreasing trend has been observed over the last decade, in the last year, the incidence of both Hepatitis has increased. The aim of this work is to compare the incidence of Hepatitis A and Hepatitis B in Galicia and in each of its provinces from 2007 to 2016. Method: Retrospective observational descriptive study. New cases of Hepatitis A and Hepatitis B where studied in Galicia and its provinces, which were notified to the “RENAVE”, from 2007 to 2016. Through the EDOs web app, data was accessed about the new annual cases in Galicia. Using Excel, were have compared this data and we have calculated the accumulated incidence which is the difference between the new cases and the average population from 2007 to 2016. Results: The incidence of Hepatitis A and Hepatitis B in Galicia has been changed throughout the study period. Although the incidence of Hepatitis B is greater than the incidence of Hepatitis A, both suffered and increase in the number of cases in the last year. At the provincial level, Hepatitis B has greater incidence than Hepatitis A in all provinces except for Lugo, where the incidence is higher for Hepatitis A. The accumulated incidence showed that the highest risk of developing Hepatitis A is in A Coruña, and the highest risk of developing Hepatitis B is in Pontevedra. Conclusions: During the last year the incidence of Hepatitis A and Hepatitis B has been increased in Galicia and its provinces. Outbreaks of Hepatitis A may be related with sexual relationships with an infected person (especially homosexuals), people traveling to areas of high endemicity without prior immunization, or familiar or cohabiting infections (including childcare centre). On the other hand, the increase in the incidence of Hepatitis B may also be due to sexual relationships with an infected person without using protection and the reused of needles and syringes (for example, tattoos or intravenous drugs users…).
