Confiabilidad y validez de las escalas de dolor neonatal en la Unidad de Cuidados Intensivos

Resumen

Objetivo principal: Analizar la confiabilidad y validez de las escalas del dolor en el recién nacido ingresado en la Unidad de Cuidados Intensivos Neonatales. Metodología: Revisión bibliográfica cuantitativa, bajo el enfoque de Enfermería Basada en la Evidencia. Se realizó una búsqueda en PubMed, Cochrane, LILACS, Scielo, Elsevier y literatura gris a través de Google Académico. Resultados principales: Se incluyeron 29 artículos de 13 países que validaron escala. Se efectúo la adaptación transcultural en 7 estudios. El alfa de Cronbach osciló entre 0.74 – 0.98 y el coeficiente de correlación intraclase entre 0.64 -1.00. Conclusión principal: No existe evidencia contundente para proponer una escala superior a otra. Sin embargo, se propone la aplicación de NIAPAS y N-PASS para dolor agudo bajo ventilación mecánica, EDIN para recién nacidos prematuros con dolor prolongado y CRIES para dolor postoperatorio.

Abstract

Objective: To analyze the reliability and validity of pain scales in newborns admitted to the Neonatal Intensive Care Unit. Methods: Bibliographic review under the Evidence Based Nursing approach in PubMed, Cochrane, LILACS, Scielo, Elsevier and gray literature through Google Scholar. Main Results: Twenty-nine articles from 13 countries that validated the scale were included. Cross-cultural adaptation was performed in 7 studies. Cronbach’s alpha ranged from 0.74 – 0.98 and the intraclass correlation coefficient between 0.64 – 1.00. Conclusions: There is no conclusive evidence to propose one scale superior to another. However, the application of NIAPAS and N-PASS for acute pain under mechanical ventilation, EDIN for preterm infants with prolonged pain, and CRIES for postoperative pain is proposed.
