Factores que influyen en la adherencia al tratamiento de pacientes con trasplante cardiaco

Resumen

Introducción: la adherencia al tratamiento farmacológico y no farmacológico se convierte en un pilar esencial para mejorar la calidad de vida de los pacientes trasplantados, disminuir las complicaciones derivadas del trasplante y aumentar la supervivencia. Objetivo: identificar los factores que influyen en la adherencia al tratamiento en los pacientes trasplantados. Material y métodos: Como diseño metodológico se realizó una revisión integrativa. La búsqueda de la literatura se realizó en las bases de datos: SciELO, Cochrane, SCOPUS y Medline. Los criterios de selección de los artículos fueron aquellos con resúmenes y textos completos, publicados en idiomas español, inglés y portugués, entre 2010 y 2019. Se encontraron 88 artículos y fueron seleccionados 23 teniendo en cuenta los criterios de validez, fiabilidad y aplicabilidad. Resultados: De los 23 artículos seleccionados, un 61% estaba escrito en inglés y un 74% se trataba de estudios observacionales de tipo descriptivo y analítico. A partir de la revisión se identificaron cuatro categorías que correspondían a los factores que estaban relacionados con el paciente, seguido por la enfermedad, el tratamiento farmacológico y la relación con el personal de salud. Los factores más frecuentes fueron los relacionados con el paciente (61%) y el tratamiento farmacológico (17%). Conclusión: El profesional de enfermería debe valorar de manera sistemática e individual las posibles barreras que pueden convertirse en un riesgo para que el paciente abandone su tratamiento

Abstract

Introduction: adherence to pharmacological and non-pharmacological treatment becomes an essential pillar to improve the quality of life of transplant patients, reduce complications derived from transplantation, and increase survival. Objective: identify the factors that influence adherence to treatment in transplant patients. Materials and methods: an integrative review was conducted as the methodological design. The literature search was carried out in the databases: SciELO, Cochrane, SCOPUS y Medline. The selection criteria of the articles were those with abstracts and full texts, published in Spanish, English and Portuguese, between 2010 and 2019. 88 articles were found and 23 were selected considering the criteria of validity, reliability, and applicability. Results: from the 23 articles selected, 61% were written in English, and 74% were descriptive and analytical observational studies. From the review, four categories were identified to be corresponding to the factors that were related to the patient, followed by the disease, the pharmacological treatment, and the relationship with the health personnel. Being factors related to the patient (61%) and drug treatment (17%) the most frequent. Conclusion: the nursing professional must systematically and individually assess the possible barriers that may become a risk for the patient to abandon their treatment
