Narcolepsy: a systematic review

Resumen

La Narcolepsia, es un trastorno crónico del sueño, que genera principalmente una excesiva somnolencia durante el día. Las personas que la padecen, tienen dificultad para permanecer despiertos durante mucho tiempo, incluso se duermen de forma repentina. La clínica, aunque es insidiosa y gradual, puede pasar desapercibida o confundirse con otros trastornos del sueño, por eso generalmente, se diagnóstica en la adolescencia, y no en etapas más tempranas. A pesar de que su causa aún se desconoce, la hipótesis más apoyada es la autoinmune, como consecuencia de la pérdida de neuronas que se produce en el sistema orexina-hipocretina, aunque también se estudian la influencia de otros factores que pudiesen intervenir en el desarrollo de la misma. Además, su tratamiento a día de hoy, se limita a aliviar los síntomas provocados por la enfermedad en cada caso, ya que la pérdida de estas neuronas es irreversible. Por ello, el objetivo principal de esta revisión sistemática, es presentar una actualización de los conocimientos más relevantes y recientes a cerca de esta enfermedad rara, que ayude a su vez a los profesionales, a abordarla de la mejor manera y, a la comunidad en general, a conocerla. Se siguió el protocolo de revisión Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) y se registró en la base de datos de PROSPERO, con el consiguiente CRD42023395971. Los resultados que se obtienen, a pesar de que son muy específicos de la enfermedad, se consiguen reflejar con la mayor correlación posible, llegando finalmente a la conclusión, de que es necesario seguir estudiando la narcolepsia infantil, así como la etiología de la enfermedad, para mejorar su diagnóstico y tratamiento, respectivamente. Así como también se resalta, la importancia del papel de enfermería en el afrontamiento de esta enfermedad.

Abstract

Narcolepsy is a chronic sleep disorder, which mainly generates excessive sleepiness during the day. Sufferers have difficulty staying awake for a long time, even falling asleep suddenly. The clinical manifestations, although insidious and gradual, can go unnoticed or be confused with other sleep disorders, which is why it is generally diagnosed in adolescence, rather than in earlier stages. Although its cause is still unknown, the most supported hypothesis is autoimmune, as a consequence of the loss of neurons that occurs in the orexin-hypocretin system, although the influence of other factors that could intervene in its development is also being studied. Moreover, its treatment is currently limited to alleviating the symptoms caused by the disease in each case, since the loss of these neurons is irreversible. Therefore, the main objective of this systematic review is to present an update of the most relevant and recent knowledge about this rare disease, which in turn will help professionals to approach it in the best way and the community in general to learn about it. The Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) review protocol was followed and registered in the PROSPERO database, with the consequent CRD42023395971. The results obtained, although very specific to the disease, are reflected with the highest possible correlation, finally reaching the conclusion that it is necessary to continue studying infantile narcolepsy as well as the etiology of the disease, in order to improve its diagnosis and treatment, respectively. The importance of the role of nursing in coping with this disease is also highlighted.
