Resumen

Introducción: La angustia moral se ha caracterizado por ser uno de los principales problemas éticos que afectan a las enfermeras en todos los sistemas de salud. Los factores que pueden desencadenar angustia moral son variables según el área donde se desempeñe el profesional de enfermería, propios del entorno clínico donde se desarrolle los planes de cuidado. Objetivo: Develar el significado de la angustia moral a partir de experiencia vivida que tiene la enfermera de hemodiálisis, durante la atención y cuidado de sus pacientes. Pacientes y Método: Estudio fenomenológico de caso. Se desarrolló una entrevista en profundidad, grabada con consentimiento de la participante, una enfermera profesional de 45 años. Se realizó análisis fenomenológico de la experiencia según método de Streubert & Carpenter. Se cauteló el rigor metodológico del estudio a través de criterios de Guba del proceso de investigación. Aspectos éticos: se aplicaron los requisitos formulados por Ezequiel Emanuel, el consentimiento informado, confidencialidad y rigor metodológico. Resultados: La experiencia de angustia moral de la enfermera de hemodiálisis, durante la atención y cuidado de sus pacientes se mostró de manera amplia, compleja y multidimensional. Se develaron seis categorías comprensivas sobre significados interrelacionadas: Situaciones que le generan angustia moral durante el cuidado del paciente, significado afectivo de la situación de angustia moral, impotencia por no poder actuar profesionalmente con autonomía, necesidad de compartir responsabilidad, necesidad de expresar emociones de angustia y necesidad de cambio. Conclusión: La comprensión develada sobre esta experiencia constituye primera aproximación que contribuye al conocimiento integral de la experiencia vivida de la enfermera frente a la angustia moral durante el cuidado de sus pacientes. Es necesario ampliarlo a una investigación con mayor número de participantes.

Abstract

Introduction: Moral distress has been characterized as one of the main ethical problems affecting nurses in all health systems. The factors that can trigger moral anguish are variable depending on the area where the nursing professional works, typical of the clinical environment where care plans are developed. Objective: To reveal the meaning of moral distress based on the experiences by the hemodialysis nurse during the care of her patients. Method: Phenomenological case study. An in-depth interview was conducted, recorded with the consent of the participant, a 45-year-old professional nurse. Phenomenological analysis of the experience was performed according to the method of Streubert & Carpenter. The methodological rigor of the study was reinforced by Guba criteria of the research process. Ethical aspects: the requirements formulated by Ezequiel Emanuel, informed consent, confidentiality and methodological rigor were applied. Results: The experience of moral distress of one hemodialysis nurse, during the care of her patients, was shown in a wide, complex and multidimensional way. Six comprehensive categories of interrelated meanings were unveiled: Situations that generate moral anguish during patient care, affective meaning of the situation of moral distress, impotence for not being able to act professionally with autonomy, need to share responsibility, need to express emotions of anguish and need for change. Conclusion: The unveiled understanding of this experience constitutes the first approach that contributes to the integral knowledge of the experience of the nurse in the face of moral distress during the care of her patients. It is necessary to extend it to an investigation with a greater number of participants.
