Síndrome de fragilidad en el adulto mayor: una revisión sistematizada

Resumen

Introducción: El envejecimiento en México va en aumento en conjunto con la aparición de síndrome de fragilidad en el adulto mayor (SF), que repercuten a estado de salud de la persona, en México existe poca evidencia acerca del manejo, atención y prevención del SF, convirtiéndose en un problema de salud pública. Objetivos: Describir la evidencia clínica disponible relacionada al SF en el adulto mayor. Metodología: Revisión sistematizada con metodología integradora, siguiendo los pasos de la Enfermería Basada en Evidencia, se utilizaron los Descriptores en Ciencias de la Salud (DeCS): Adulto mayor” y "Síndrome de Fragilidad", términos Medical Subject Headings (MeSH): "Aged", "Frailty Syndrome" se utilizó el operador booleano AND. Búsqueda de evidencia científica en bases de datos LILACS, CUIDEN, SciELO, y PubMed, se incluyeron artículos en idioma español, inglés y portugués entre el año 2015 y 2020 y se utilizó la escala SIGN para determinar el NE/ GR. Resultados: se encontraron 51 artículos; prevalecieron estudios descriptivos (47%), metaanálisis (19.6%) y revisiones sistemáticas (9.8%), el idioma que prevalece es el inglés (54.3%), el país que más publica es Brasil (39.2%) y el Índice de Fragilidad de Groninguen es el instrumento más utilizado (23.5%). Se identificaron 6 categorías sobre SF: definición, factores de riesgo, instrumentos de evaluación, complicaciones, recomendaciones para la atención y tratamiento. Conclusión: Es necesario realizar una evaluación geriátrica integral para identificar el riesgo de presentar SF y de esta manera desarrollar nuevas estrategias para promover el cuidado, prevención, tratamiento y manejo de los adultos mayores

Abstract

Introduction: Aging in Mexico is increasing in conjunction with the appearance of frailty syndrome in the elderly (FS), which have repercussions on the health status of the person, in Mexico there is little evidence about the management, care and prevention of FS, becoming a public health problem. Objectives: To describe the available clinical evidence related to FS in the elderly. Methodology: Systematized review with integrative methodology, following the steps of Evidence-Based Nursing, using the Health Sciences Descriptors (DeCS): “Adulto mayor” and “Síndrome de Fragilidad”, Medical Subject Headings (MeSH) terms: “Aged”, “Frailty Syndrome”, the Boolean operator AND was used. Search of scientific evidence in LILACS, CUIDEN, SciELO, and PubMed databases, articles in Spanish, English and Portuguese between 2015 and 2020 were included and the SIGN scale was used to determine the NE/GR. Results: 51 articles were found; descriptive studies (47%), meta-analysis (19.6%) and systematic reviews (9.8%) prevailed, the prevailing language is English (54.3%), the country that publishes the most is Brazil (39.2%) and the Groningen Frailty Index is the most used instrument (23.5%). Six categories of FS were identified: definition, risk factors, assessment instruments, complications, recommendations for care and treatment. Conclusion: It is necessary to perform a comprehensive geriatric assessment to identify the risk of presenting FS and thus develop new strategies to promote care, prevention, treatment and management of older adults
